In addition, we will ensure that you have all of the proper forms required to complete the medical portion of your application. Dr. Sabido will give you an immigration physical examination, shot record review and/or vaccination administration, and then provide you with a completed Form I-693 to go with your change of status application.

About Civil Surgeons:

Most applicants for adjustment of status are required to have a medical examination. The medical examination must be conducted by a Certified Civil Surgeon who has been designated by the United States Citizenship and Immigration Services. Immigration regulations state that each Civil Surgeon selected shall be a licensed physician with no less than 4 years of professional experience. Dr. Sabido has met all of the requirements and is a Certified Civil Surgeon.

Vaccine Requirements:

United States law requires that all applicants for lawful permanent residence obtain certain vaccinations prior to a status change. Our office will review your vaccination records and administer any vaccines required based on your age. Please bring any shot records from a prior physician with you to your appointment.

Required Vaccines Include:

  • Hemophilus Influenza Type B (Hib)
  • Measles
  • Pertussis
